Following a review of the proposal, the researcher will receive a copy of the Approval Form indicating whether or not the proposal was approved.

Parental Consent

If student questionnaires or interviews are to be administered, or if confidential data are requested, written, active, informed parental consent must be obtained for each student prior to participation in the study. Attached is a sample consent form. This sample may be used as a guide in developing consent forms required for the proposed study. A copy of any instrument(s), which are not commercially developed, must be sent to parents/guardians with the consent form.

School Climate Survey

Several researchers have been interested in our MCS School Climate Survey as a potential source of data for their projects.  Copies of the three forms of the 2007 survey (student, parent and staff) are provided here for your information.
